Eligibility and Sign-Up Information
Athletic Eligibility Overview
In order to be eligible to practice and compete in athletics at Lake Mills High School, students must meet all eligibility requirements and have all required athletic items completed and turned in to the high school athletics office for the current school year. Read more about what is required for sign-up in the "Sign-Up Information" section below.
Academic Eligibility:
4th Quarter grades set fall start eligibility.
2 F's = Ineligible
1 F + below 1.5 GPA = Ineligible
***All freshmen start the school year with a clean slate (8th grade 4th quarter grades will not make them ineligible).
Athletes must maintain a 1.5 GPA.
If a student is getting a D or F when progress reports are run, they must attend athletic study hall twice a week after school for 30 minutes.
If a student fails two or more classes for the quarter they are ineligible to participate in their sport for 15 school days.
General Eligibility Requirements:
All student-athletes must complete the following sign-up process before being eligible to participate in athletic practices or events:
1. Online sign-up paperwork in Skyward Family Access (opens in the summer for the upcoming school year- specific dates that forms open are sent to families via email).
2. Turn in supporting items, including participation fees, physical or alternate year physical form, attending or viewing the mandatory parent/student-athlete meeting, and concussion testing (if required for their sport).
Other Eligibility Requirements:
Transfer students need to set up a meeting with the athletic director to complete the eligibility questionnaire required by the WIAA.
Students must be in attendance for the entire school day to be eligible to practice or play in a game unless they are excused for an appointment or administratively approved absence.

Step One: Online Sign-Up in Skyward Family Access
Online sign-up forms will be available in Skyward during mid-summer each year for the next school year, and will remain available until the start of the spring sports season.
Forms in the sign-up include:
- Athletic Permission Form/Athletic Information Card
- Co-Curricular Code Form/Authorization for Release of Medical Information
- Concussion and Sudden Cardiac Arrest Forms
Step Two: Supporting Items
- Participation Fee - $50 per sport ($250 cap per family per year). Fees will be available to pay within 48 hours of completing the online sign-up. Payments can be made online in Skyward Family Access, or turned into Ms. Berndt. In-person payments must be made with exact cash or a check written out to Lake Mills Area School District (LMASD).
- Attend the Mandatory Parent/Student-Athlete meeting on July 29th, 2025 at 6:30 PM in the High School Auditorium. If you were unable to attend, please watch a recording at this link to satisfy this requirement. There is a QR code within the video used to record participation.
- Complete SWAY concussion baseline testing, if required. Check out our athletic trainer's page for more information!
- Turn in an up to date physical or alternate year physical card.
*To see if your student-athlete needs a Physical or Alternate Year Form, check the Portfolio Tab in Skyward Family Access. This will show you what documents were most recently submitted.
**Physicals are good for two school years if the date of examination is after April 1st. A physical dated before April 1st is good for the rest of that school year and the following one. For this school year, a physical must be dated on or after April 1, 2024 to be valid.
